Wanky MD Plus controller?

I have one particular MD Plus controller that occasionally drops out pieces of equipment, especially if I reboot the Pro-Plus. I notice in Diagnostics, under "Restart", that the "Cold Restart State" = NO while the "User Restart State" = YES. The cold restart source is "Controller". I have downloaded both the Cold Restart and setup data but there's no difference after doing so. All of my other controllers (23 more) all show "Cold Restart State" = Yes and "User Reset State" = NO and they exhibit no problems. The controller firmware versions are all the same at hardware rev 5.17 and software rev 11.3. Controller idle time is ~ 80%. I'm not even sure if the observation I have described here is related to my equipment dropping out, as the equipment failures flag outputs on one particular DOUT card, which I've replaced in the past several weeks.

  If you swap controllers (one from the good 23) does the problem stay with the equipment or move with the controller?
  
  Are these simplex controllers? Vertical or horizontal carriers?

  Is your one particular DO card redundant? If yes then does the card show that standby is available in diagnostics?
