Alfred offers a valid solution. There are many variants possible that will yield the same result.   You could also simply use two C&E blocks and combine their effecst in an OR gate to get the same result.  

Another approach might be to group your causes in a logical manner, such that their effect represents that group of causes.  Say five causes come from inlet pressures.  6 more come from another related grouping.  You might have 3or 4 C&E blocks that feed a final block that combines these "grouped" causes with remaining causes such that the final C&E requires less than the maximum 16 Causes.

By breaking the problem down into several groupings, it might result in a cleaner FB diagram and provide indications for displays and such.
