Posible Carrier problem due to plugging 120 VAC to a DO (I/O) card.

Hello, I´m haveing problems trying to identify a failure.

Recently a contractor connected a 120VAC circuit to a digital output channel intending to use it as a dry contact.

The digital output is a High side 24vcd output card Series 2

At the biggining nobody detected this issue and also we started to make running tests activating this output channel.We just noticed that the motor was driven and started directly just by conecting the circuit to the channel even if the channel was not activated.

Then, after a few tests one AI card connected on the same carrier was damaged. 

I changed that AI card (wich is now working ok), and at that moment noticed that wrong connection. Apparently there was no problem with the DO card but just in case changed it for a new one. What we notice is that even i Have the output activated I don´t have the 24 vcd voltage at the terminal block. I already check ok the fuse in the block terminal an verified ok the input voltaje to the carrier. I also canged tha DO card to differente positions in the same carrier end tested with the same result.

Is ther another cause for what I am not having the output voltaje from the DO card? What other tests can I make? 

The DI, AO and AI cards conected in the same carrier are working ok.
